Theatre Permission Form

Hello everyone!
I hope it was a great first week of school. On the first day, students received a permission form to see, Amelia Earhart at the Children's Theatre of Charlotte on Friday, November 13th from 10:45am-2:15pm. Permission slips are due on Friday, September 18th. Cash with exact change, Check or online payments are accepted. However, online payments are an extra $1.00 plus an additional processing fee. The trip is $16 for students and $14 for chaperones. Payments are non-refundable.
We could definitely use chaperones! If you're interested please complete the volunteer application and submit your clearance verification to me before paying for a ticket:
